New Forest Dementia Hub at Linden House Care Home Lymington, Wednesdays 2pm

 

New Forest Primary Care Network is a relatively recently established collaborative healthcare network to improve community health and provide expanded services across the region. Wistaria and Milford Surgeries, and Chawton House Surgery are part of it, along with Lyndhurst Surgery and New Forest Medical Group. There is a good deal of very useful information on its website including a specific section about dementia (https://newforestpcn.co.uk/dementia).

New Forest PCN Dementia Hub

Our Dementia Hub provides a safe, welcoming, and supportive space for people living with dementia, as well as their families, friends, and carers.

Whether you're looking for practical advice, information about local services, or simply the opportunity to connect with others who understand your experiences, you'll find a friendly and relaxed environment where everyone is welcome.

If you're attending with someone living with dementia, they can take part in a supervised activity session while you access support, information, and conversation.
